Eight dimensions in medication continuum are identified: patients’ participation, support in decision taking, trans-border continuity, coordination of activities, communication with patient, problems solution, flexibility and accessibility, focusing on patient. The study offers an approach to evaluation of medication continuity on the basis of patient-centricity.
